How can I control and prevent algae and cyanobacteria in my planted aquarium?

I have a decent 14 gallons U.S. planted tropical aquarium. The algae is bad enough, but cyanobacteria is terrible. It is in the decoration, gravel, plants, etc. Whenever I lose, it comes back. Any advice? This is my tank: http://www.youtube.com/watch?v=eyDMieKzs6s&feature=channel_page

You need to reduce nutrients. Unknowingly, I'll say that your tank have levels of nitrates and phosphates that are excessive. Levels should be checked and then work on a solution to eliminate these nutrients. swimmer
